    Seroprevalence Of Hepatitis-b Virus In Mid And Far Western Region In Nepal

    Hepatitis B is significant health problems that might involve the late sequel of liver cirrhosis and hepatocellular carcinoma. The present study aimed to know the seroprevalence of hepatitis B virus (HBV) in mid and far western region in Nepal with various clinical conditions.This was a retrospective study conducted in mid and far western region in Nepal, which was performed in the Central Laboratory of Microbiology at Nepalgunj Medical College and Teaching Hospital, Banke, Nepal during the period of September 2010 to April 2012. The serum samples were tested for Hepatitis B surface Antigen (HBsAg) by Sandwich immunoassay. Total 7010 patients including 43.72% male and 56.28% female were tested for HBsAg. Of them, 135 were positive and 6875 were negative.In 135 positive cases 84 (62.22%) were male and 51 (37.77% )were female. In 6875 negative cases 2981 were male and 3894 were female. The seroprevalence rate of HBV was 1.93% in mid and far western region in Nepal. Seroprevalence of HBV seems to be higher in male then the female; it was 2.75% in male and 1.29% in female.The study revealed that the seroprevalence of HBV was alarmingly higher in such a population, which probably reflects a high background prevalence of HBV infections should be taken into consideration and Implementation of community-based preventive measures and improved strategies for safe blood supply might prove useful to decrease the seroprevalence

    Human behaviour and occupancy accounts for a substantial proportion of variation in the energy efficiency pro le of domestic buildings. Yet while people often claim that they would like to reduce their energy bills, rhetoric frequently fails to match action due to the effort involved in understand- ing and changing deeply engrained energy consumption habits. Here, we present and, through dedicated experiments, test in-house developed soft-ware to remotely identify appliance energy usage within buildings, using energy equipment which could be placed at the electricity meter location. Furthermore, we monitor and compare the occupancy of the location under study through Ultra-Wideband (UWB) radar technology and compare the resulting data with those received from the power monitoring software, via time synchronization. These signals when mapped together can potentially provide both occupancy and speci c appliances power consumption, which could enable energy usage segregation on a yet impossible scale as well as usage attributable to occupancy behaviour.     Gastrointestinal Bleeding in Patients With Acute Respiratory Distress Syndrome: A National Database Analysis.

    Background:The goal of our study was to determine the impact of gastrointestinal bleeding (GIB) on in-hospital outcomes among acute respiratory distress syndrome (ARDS) patients, and subsequently determine the potential risk factors for the development of GIB. Methods:ARDS patients with and without GIB were identified using the National Inpatient Sample (2002 - 2012). Linear regression analysis was used to assess impact of GIB on in-hospital mortality, length of stay and total charges. Univariate logistic regression was used to determine associated odds ratios (OR) for causes of ARDS and common comorbid conditions. Results:We identified 149,190 ARDS patients. The incidence of GIB was the highest among patients \u3e 60 years (P \u3c 0.001). GIB was associated with longer hospitalization days (7.3 days versus 11.9 days, P \u3c 0.001), higher mortality (11% versus 27%, P \u3c 0.001) and greater economic burden (82,812versus82,812 versus 45,951, P \u3c 0.001). GIB was common in cirrhosis (OR: 8.3), peptic ulcer disease (OR: 3.7), coagulopathy disorders (OR: 3.003), thrombocytopenia (OR: 2.6), anemia (OR: 2.5) and atrial fibrillation (OR: 1.5). ARDS secondary to aspiration pneumonia (OR: 2.0), pancreatitis (OR: 2.0), sepsis (OR: 1.6) and community acquired pneumonia (OR: 0.8) was more likely to have GIB. Conclusion:Our study demonstrates that GIB in ARDS patients is associated with significant increased mortality, hospitalization and health care cost

    Inhibition and kinetic studies of lignin degrading enzymes of Ganoderma boninense by naturally occurring phenolic compounds

    Aim Lignolytic (lignin degrading) enzyme, from oil palm pathogen Ganoderma boninense Pat. (Syn G. orbiforme (Ryvarden)), is involved in the detoxification and the degradation of lignin in the oil palm and is the rate‐limiting step in the infection process of this fungus. Active inhibition of lignin‐degrading enzymes secreted by G. boninense by various naturally occurring phenolic compounds and estimation of efficiency on pathogen suppression was aimed at. Methods and Results In our work, 10 naturally occurring phenolic compounds were evaluated for their inhibitory potential towards the lignolytic enzymes of G. boninense. Additionally, the lignin‐degrading enzymes were characterized. Most of the peholic compounds exhibited an uncompetitive inhibition towards the lignin‐degrading enzymes. Benzoic acid was the superior inhibitor to the production of lignin‐degrading enzymes, when compared between the 10 phenolic compounds. The inhibitory potential of the phenolic compounds towards the lignin‐degrading enzymes are higher than that of the conventional metal ion inhibitor. The lignin‐degrading enzymes were stable in a wide range of pH but were sensitive to higher temperature. Conclusion The study demonstrated the inhibitor potential of 10 naturally occurring phenolic compounds towards the lignin‐degrading enzymes of G. boninense with different efficacies. Significance and Impact of the Study The study has shed a light towards a new management strategy to control basal stem rot disease in oil palm. It serves as a replacement for the existing chemical control

    Automatic User Preferences Selection of Smart Hearing Aid Using BioAid

    Noisy environments, changes and variations in the volume of speech, and non-face-to-face conversations impair the user experience with hearing aids. Generally, a hearing aid amplifies sounds so that a hearing-impaired person can listen, converse, and actively engage in daily activities. Presently, there are some sophisticated hearing aid algorithms available that operate on numerous frequency bands to not only amplify but also provide tuning and noise filtering to minimize background distractions. One of those is the BioAid assistive hearing system, which is an open-source, freely available downloadable app with twenty-four tuning settings. Critically, with this device, a person suffering with hearing loss must manually alter the settings/tuning of their hearing device when their surroundings and scene changes in order to attain a comfortable level of hearing. However, this manual switching among multiple tuning settings is inconvenient and cumbersome since the user is forced to switch to the state that best matches the scene every time the auditory environment changes. The goal of this study is to eliminate this manual switching and automate the BioAid with a scene classification algorithm so that the system automatically identifies the user-selected preferences based on adequate training. The aim of acoustic scene classification is to recognize the audio signature of one of the predefined scene classes that best represent the environment in which it was recorded. BioAid, an open-source biological inspired hearing aid algorithm, is used after conversion to Python. The proposed method consists of two main parts: classification of auditory scenes and selection of hearing aid tuning settings based on user experiences. The DCASE2017 dataset is utilized for scene classification. Among the many classifiers that were trained and tested, random forests have the highest accuracy of 99.7%. In the second part, clean speech audios from the LJ speech dataset are combined with scenes, and the user is asked to listen to the resulting audios and adjust the presets and subsets. A CSV file stores the selection of presets and subsets at which the user can hear clearly against the scenes. Various classifiers are trained on the dataset of user preferences. After training, clean speech audio was convolved with the scene and fed as input to the scene classifier that predicts the scene. The predicted scene was then fed as input to the preset classifier that predicts the user’s choice for preset and subset. The BioAid is automatically tuned to the predicted selection. The accuracy of random forest in the prediction of presets and subsets was 100%. This proposed approach has great potential to eliminate the tedious manual switching of hearing assistive device parameters by allowing hearing-impaired individuals to actively participate in daily life by automatically adjusting hearing aid settings based on the acoustic scen

    Surgical ligation of scrotal varicocele for male factor infertility is a valid option of treatment

    Objective: To evaluate the role of surgical ligation of scrotal varicocele for treatment of male factor infertility. Methods: We studied 60 patients who presented with infertility and were also found to have scrotal varicocele. Patients with other probable causes contributing to infertility were excluded. Diagnosis was made by clinical examination and scrotal ultrasonography. All the patients underwent either laparoscopic varicocelectomy or open retroperitoneal high ligation of the testicular veins. Operative time and hospital stay was recorded. All patients were evaluated for postoperative improvement of semen parameters and development of any postoperative complication. The results were analyzed by commercially available software. Results: During the study period 5 patients were lost to follow up and were excluded. The rest of patients (n=55) were young, with age ranging from 20-35 years (Mean age 27.8 years, SD +/- 4.38). All patients had unilateral left sided varicocele; two patients (3.6%) had grade I varicocele, 21 patients (38.2%) had grade II varicocele and 32 patients (58.2%) had grade III varicocele. Statistically significant improvement in sperm density (p value \u3c 0.05), sperm activity (p value \u3c 0.05) and sperm morphology (p value \u3c 0.05) was observed after the surgical ligation. The mean operative time was 54.88 minutes (SD + 13). The mean hospital stay in laparoscopic procedure was 33.4 (SD + 15.3). Minor complications were noted in 13 patients and included superficial wound infection in 3 patients, 6 patients exhibited testicular pain and persistence of varicocele in 4 patients. Conclusion: Surgical ligation of scrotal varicocele is asafe and effective mode of treatment of male factor infertility in selected population

    High- and low energy nonthermal X-ray emission from the cluster of galaxies A 2199

    We report the detection of both soft and hard excess X-ray emission in the cluster of galaxies A 2199, based upon spatially resolved spectroscopy with data from the BeppoSAX, EUVE and ROSAT missions. The excess emission is visible at radii larger than 300 kpc and increases in strength relative to the isothermal component. The total 0.1-100 keV luminosity of this component is 15 % of the cluster luminosity, but it dominates the cluster luminosity at high and low energies. We argue that the most plausible interpretation of the excess emission is an inverse Compton interaction between the cosmic microwave background and relativistic electrons in the cluster. The observed spatial distribution of the non-thermal component implies that there is a large halo of cosmic ray electrons between 0.5-1.5 Mpc surrounding the cluster core. The prominent existence of this component has cosmological implications, as it is significantly changing our picture of a clusters's particle acceleration history, dynamics between the thermal and relativistic media, and total mass budgets.Comment: Accepted for publication in Astrophysical Journal, Letter

    Septic emboli of the lung due to Fusobacterium necrophorum, a case of Lemierre\u27s syndrome

    © 2019 The Authors Fusobacterium necrophorum plays a causal role in a rare and life-threatening condition, Lemierre\u27s syndrome. It is characterized by infection involving the posterior compartment of the lateral pharyngeal space complicated by septic suppurative thrombophlebitis of the internal jugular vein with F. necrophorum bacteremia and metastatic abscesses, primarily to the lung and pulmonary septic emboli. Herein, we present a very rare case of oropharyngeal infection complicated by Lemierre\u27s syndrome with characteristic septic emboli to the lungs presenting as sore throat in a previously healthy patient. A 23-year-old woman presented with sore throat and was found to be in sepsis and acute kidney injury. She was found to have septic emboli in lung and Streptococcus anginosus and F. necrophorum in blood. She was diagnosed with Lemierre\u27s syndrome and successfully treated with antibiotics. Lemierre\u27s syndrome should be included in the differential diagnosis in young patients who deteriorate in the setting of a sore throat. If the suspicion is high, throat swabs from young patients with nonstreptococcal group A tonsillitis should be cultured anaerobically on selective medium to detect the presence of F. necrophorum. While clinicians of the infectious disease team may be familiar with this condition other departments including internal medicine and critical care team may less so. Unless clinicians are aware of this syndrome, diagnosis and treatment can be delayed leading to higher morbidity and mortality
